Instructions

• 1. Fill out the form

• 2. Save your progress when you need a break

• 3. login and click on the form to pick up where you left off

• 4. Complete final certifications and click submit*

Loading a Saved Form

• Open the form you would like to load

• under “load saved progress” you will see the dates you have saved the form

• Hover to the right of the date you want to load to show the “load” button

• Click “load” to pick up where you left off
